Abstract

Decentralization in the health area in 2001, demended the health sector in district and cities in Indonesia to be able to carry the main functions, namely : - stewardship (oversight); - creating resources (investment and training); - financing (collecting, pooling and purchasing); -delivering services. According to WHO, 2000: the objectives of the health system include: - improved health status; - responsiveness (to people?s non-medical expectations); - fair (financial) contribution. Since the assessment of health systems performance and achievement of obligatory public health functions and minimum service standard of the health sector have never been conducted, the study was conducted in six districts and cities with different level of Human Development Index and Fiscal Capacity. Data and information were collected cross-sectionally. Primary data were collected from the visitors of health facilities and secondary data were obtained from the existing reports/records of the health facilities and health offices; as well as from the National Socio-Economic Data. Conceptual framework on Health Systems Performance Assessment developed by the WHO (2000) have been used. The results show that modifications need to be applied in the instrument, such as in the assessment of Fairness in health care financing. The assessment also shows which component need to be improved, in order to improve the performance of health system. Assessment of the achievement of obligatory public health functions and minimum service standard of the health sector shows that all sampled district/cities have implemented the functions and standards as agreed and achieve some of the targets for Health Indonesia 2010; although some health efforts need to be enchanced and given more priorities. Validation of data on achievement of obligatory public health functions and minimum service standard of the health sector can be carried out by analyzing other sources of data, such as community based data: SUSENAS (Nasional Socioeconomic Survey, SURKESNAS (National Health Survey), SDKI (Demographic Health Survey). Main functions of the health sector have been carried out with different quality. In general the health sector in the sampled districts/cities have developed the strategic plan and the annual plan. As a conclusion, the assessment can be carried out periodically, to improve allocation and distribution of resources as needed and as a base for advocacy activities and setting priorities.
