Abstract

Background: in Puskesmas of Pekalongan Kabupaten medicines are delivered free of charge to patients, but the patients paid Rp 400,- as administration fee. Puskesmas is one of public health services run by MOH The patients were examined by the medical personnel and given medication which were prescribed. The drugs are supplied to Puskesmas by District Drugs Warehouse (GFK), 80% procured by the central government / INP RES and 20% by the local government. The central government only procures generic drugs. An important point is that the government subsidy was mainly intended for the poor, but the Puskesmas facility actually receive people of all income levels, not only the poor but also the middle class and rich people. This research adopted the manual of Rapid Pharmaceutical Management Assessment an Indicator Based Approach (PAHO 1995). The scope of indicators of drugs cost analysis consisted of Ministry of Health Budget and Finance, Ministry of Health Pharmaceutical Procurement,. Ministry of Health Pharmaceutical Logistic, and Patient Access and Drugs Utilization. Result: the expenditure on pharmaceutical per capita from MOH budget in Kabupaten Pekalongan before the crisis (1994-1997) was approximately Rp 876,- /capita and after the crisis increased to Rp 1.897,-/capita, while prices of the drugs increased up to approximately 2--4 times. The set indicator drugs in GFK, puskesmas and RSU (district hospital) storage were never out of stock. Average number of medication prescribe per curative (ISPA) is 4,0 item (in the manual is 2,0 item); average ratio of antibiotic prescription in Puskesmas is 91% and in RSU is 97% (in the manual is 61%). Average ratio of generic drugs in prescription in puskesmas is 100%, but in RSU is only 32%.
